Why doesn’t Second Life offer a prepaid debit card for Linden Dollars?

Because Linden Dollars is not a universally accepted or traded currency as far as financial institutions are concerned. It wouldn't make much sense for VISA or Mastercard to make a product that is specific to cashing out from SecondLife. LindeX is not widely accepted as an exchange to bet on, albeit PayPal is one of the few companies that does work in taking Linden Dollars - that are converted to US Dollars (via LindeX) and cash out.

I guess once the demand for Linden Dollars cashing out becomes large enough, who knows, maybe VISA and Mastercard both will pounce on the opportunity. However, I wouldn't be surprised if some third party will introduce a Prepaid card that would allow you to do the same.
