Which reliable financial institutions provide offshore or anonymous debit cards?

I doubt you would be able to find pre-paid debit cards off-shore that are designed to be anonymous in nature. Speaking from South East Asia / South Asia point of view, the banking industry is quite regulated (contrary to popular belief otherwise). No issuer is going to be able provide prepaid cards (especially Visa/Mastercard) debit cards that are tied into anonymity.
