Bank of America Merchant Account. Hands down!
One of the best service out there for international use. The caveat being, international usage assume, International customers, charging their international cards, but the base currency is US Dollars.
Rarely came across a country and/or card that was not accepted (barring countries and/or banks in flagged list). Very good fraud management system as well. The most epic factor was their back-office and how they are willing to work with you to help you settle charges, reverse charges, authorisation errors, etc.
