Which banks offer the best travel-friendly banking (no FX fees, no international fees, no ATM fees)?

I’ve been following banks for a long long time to see if there is indeed a bank that has no international fees and no ATM fees.

Barring same bank (like say using a Citibank network, which might have lower fees but not zero fees), I have yet to come across a bank like that.

When a Bank in US (for example) has an account holder uses a card in some foreign country at an ATM, the fees is levied by the bank that owns that ATM in the foreign country and not the US bank. Essentially, the Bank in the US is helpless when it comes to the fees that their customers will pay when using their ATM card on an ATM outside the US.
