If you're a foreigner, you need a passport and would need to visit the bank. If you have previous or existing banking relationship with any of their partner or recognized banks, then, that is good, else, they might ask for a reference (though few do now a days).
You can deposit cash into your account (I think the limit is Euros 10,000 equivalent). Anything more, and you would need to show source of funds. For incoming wire transfer, or cheque of another bank, you would need to sign a wavier that the money is not illegal money, etc. and sign-off on the source of funds declaration.
If you open an account with a representative of Swiss banks or a Swiss bank branch outside the country, then needless to say, a visit to the bank within Switzerland is not required.
