Varies from start-up to start-up, but from what I have seen, the person is the Head of Technology.
They are technically inclined/proficient.
They are tasked to do product / service development (from a technical point of view, and oversee it as well). This could mean hands on coding (if required), technical integration, systems integration, server/network management, etc.
A CTO is to whom a CEO usually goes to and informs him/her what needs to be done to get clients or go to market, and the CTO then knows what needs to be done at the back-office to execute that strategy.
