Has anyone found a fully online-only bank that caters specifically to small and medium enterprises (SMEs)?

Other than the one (and only) branch which is their head-office, you might want to look up USAA (USAA / Welcome to USAA) - the Bank is almost entirely virtual (though they have a lot of physical point of presence by association for things like cheque deposits, etc.)
